A written report related to the patient and/or the treatment, care, observation or other dental/medical activities performed, prepared in the following cases:

  • Need for a change in the treatment and/or financial plan;
  • Inability to complete treatment due to patient non-appearance or other dental, medical, technical or personal reasons related to the patient;
  • Complete or partial refusal of treatment;
  • Temporary or permanent cessation of the practice of dental medicine and need for redirection;
  • Clarification of an emerging situation.
  • This report is not intended for the physician directly involved in the care of the patient.
